To create a provider account, go to the MPIPHP home page and click on “Sign Up” button. You must read and agreed to the Terms of Use and Privacy Policy in order to continue. If you agree, click “Submit” button. (Clicking the Submit button indicates your agreement with the terms spelled therein.) Select “Provider” on the pull-down menu, and then follow the directions on the following screens. After you complete the sign-in process, you will receive an E-mail confirmation.
