Retiree Plan The Retiree Health Summary Plan Description (SPD) has been organized to make the information more understandable and easier to access. The Supplement is located at the end of the SPD document. For your convenience, we offer the Retiree SPD in Adobe Acrobat Portable Document Format (.pdf) for your immediate download. If you are enrolled in Health Net or Kaiser your Summary Plan Description will not be complete without your plan's Evidence of Coverage. Also, if you belonged to a group of Participants who merged into the MPI Health Plan, your Summary Plan Description will not be complete without your specific group's SPD Appendix.
