Active Plan The Active Health Summary Plan Description (SPD) has been organized to make the information more understandable and easier to access. We’ve made the SPD fully interactive. Also, some of the more commonly used forms are attached to the end of the document. For your convenience, we offer the Active SPD in Adobe Acrobat Portable Document Format (.pdf) for your immediate download. To download, simply click on the SPD icon at right. (You will need the free Adobe Acrobat Reader to view the document. Current version is 8.) If you are enrolled in Health Net, Kaiser, or the Oxford Health Plan, your Summary Plan Description will not be complete without your plan's Evidence of Coverage. Also, if you belonged to a group of Participants who merged into the MPI Health Plan, your Health Summary Plan Description will not be complete without your specific group's SPD Appendix.
