ACA Health Coverage IRS Tax Forms - Participants

In early February, you may receive one or more forms providing information about the health care coverage that you had or were offered during the previous year. Much like Form W-2 and Form 1099, which include information about the income you received, these new health care forms provide information that you may need when you file your individual income tax return.

Starting in February, and every year moving forward, MPI will be sending participants 1095B forms as proof of health insurance coverage for each appropriate tax filing year. If you have any questions specifically about filing this form, please refer to the IRS website or your financial advisor for assistance with completing your personal tax return.

More information about these forms can be found on the IRS Website at this link:

UCLA Health
UCLA Health Cyber Attack

On July 17, 2015, UCLA Health revealed that it had been the victim of a cyber attack. At this time, while it is possible, there is no evidence that the attackers actually accessed or aquired any individual's personal or medical information. UCLA is working with the Federal Bureau of Investigation and has launched its own investigation.

In addition, UCLA has set up a website (www.myidcare.com/uclaprotection) with information about the attack. If you prefer to speak to someone about the incident, please call UCLA at 877-534-5972 Monday through Friday between the hours of 6 am to 6 pm pacific time.

Anthem Blue Cross

Anthem Blue Cross was recently the target of a sophisticated cyber attack that compromised the personal information of many MPI Participants. Some of Anthem’s partner and affiliated health plans across the country were also affected. To date, these include:

  • Blue Shield (Blue Cross/Blue Shield companies in Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, New York, Ohio, Virginia, Wisconsin)
  • Premera (Premera Blue Cross, Premera Blue Cross Blue Shield of Alaska and affiliate brands Vivacity and Connexion Insurance Solutions, Inc. in Alaska, Oregon and Washington)
If your personal information was compromised, you will receive a letter from Anthem - or from its affiliates - with specific instructions about how you may receive free Identity Repair services or sign-up for free Credit Monitoring.

For more information regarding the cyber attack, contact Anthem Blue Cross

Phone: 1-877-263-7995

MPI will continue to provide additional information as it becomes available.
